Use this checklist to prepare for your consultation, not to replace personal discharge instructions. Request records in a language your home healthcare team can use, and agree care responsibilities before travelling.

Discharge is not automatic clearance to fly.
Plan the return flight around your clinical assessment and the airline’s requirements. Air travel and surgery can increase blood-clot risk; discuss timing and individual precautions with your clinical team.
Follow-up does not end with a support package.
Bariatric surgery requires ongoing reviews and nutritional monitoring. Agree the necessary tests and follow-up arrangements with your home healthcare team. If you suspect a complication, seek local medical assessment promptly; do not wait for a portal or message reply.
